Your enrollment in these plans will continue until the last day of the month in which your employment is terminated. Shortly after your termination is processed, Pacsun’s COBRA administrator will send an enrollment packet to your home address. You will have the option to enroll yourself or your covered dependent up to 18 months of coverage.

Enrollment in any Flexible Spending Account will end effective as of the date of the termination of your employment. Shortly after your termination, you will be solicited for continuation of coverage under the provisions of COBRA for Health Care Flexible Spending Account. Continuation of coverage for Dependent Care Flexible Spending Accounts are not allowed under COBRA.
You may only be reimbursed for claims incurred while you were active and enrolled in the FSA. Therefore, unless you elect COBRA, any claims incurred after your date of termination will not be eligible for reimbursement. In addition, all claims must be submitted for reimbursement within 30 days from your date of termination.
Depending on your 401(k) Plan account balance, you have various options regarding your retirement funds after employment ends. Your coverage for all life insurance and AD&D coverage will discontinue effective as of your date of termination. You have portability rights for this coverage and may be able to continue coverage under an individual policy with New York Life. To do so, you must contact New York Life to complete an application within 31 days of termination.
Coverage for short-term and long-term disability will discontinue effective as of the date of termination. There are no conversion or portability rights for this coverage.
Your coverage for the above voluntary policies will discontinue effective as of your date of termination. You have portability rights for this coverage and may be able to continue coverage under an individual policy with Aetna. To do so, you must complete the Aetna Voluntary Portability Form and submit to Aetna within 31 days of termination.
